Cemetery Flag Waiver Form

The below Summit County Veterans Organization hereby submits this waiver application for Memorial Day Observance Cemetery flags, markers, and post hole diggers in lieu of the sum of five hundred dollars ($500.00) as outlined in ORC 307.66.

Deadline for the return of all waivers for ordering is by 8/17/2022. ANY ORDERS PLACED AFTER THAT WILL NOT BE GUARANTEED IN TIME FOR MEMORIAL DAY!

Memorial Day supplies will be available for pick up at the Veterans Service Commission beginning the week of Monday April 11ththrough Friday April 21, 2023. Distribution hours will be as follows: 10:00 am to 12:00 noon and 1:00 pm to 2:30 pm on Monday through Friday at 1060 E Waterloo Rd. Akron, Ohio 44306
